c)  Your previous and current medical history, provided by you or your GP.

d)  The revised version of the contract terms and conditions for the healthcare/holistic services we provide.

e)   Your bank details/credit/debit card if you register.

f)  Information about your marital status, next of kin, dependants nominated, and emergency contacts.

g) Information about any medical or health conditions, including disabilities that require reasonable adjustments by “to-Nirvana”.

h) “To-Nirvana” collects equal opportunity monitoring information, including ethnic origin, sexual orientation, and health and this information may be collected in various ways.  Additionally, “to-Nirvana” may obtain personal data about you from third parties, such as other healthcare providers.

PROVIDING YOUR PERSONAL DATA

We will inform you if providing personal data is optional.  In all other cases, we need it to provide care and treatment to you.

MONITORING OF COMMUNICATIONS

We may monitor communications related to our dealings with you to ensure an appropriate standard of care, comply with regulations, prevent and detect crime, protect our systems, and maintain quality control.  We may also monitor activities on our network and systems for these reasons.

We use your personal data to provide healthcare/holistic support in the following ways:

  • To determine the best treatment for you and support your healthcare.

  • To ensure we take the necessary steps during your treatment and maintain up-to-date records.

  • For effective governance, accounting, and management of our business operations both internally and by third parties.

  • For helpful feedback on client experience and quality of care.

  • To monitor emails, calls, other communications and other activities on the “to-Nirvana” network and systems.

  • For market research, surveys, analysis, and developing statistics to improve clinical performance, as well as to comply with legal obligations.

  • When you exercise your rights under data protection law and make requests.

  • For compliance with legal and regulatory requirements making related disclosures.

  • For the establishment and defence of legal rights.

Based on your consent:

  • With your next of kin or any other person, you nominated to be contacted in case of an emergency.

  • If you request us to disclose your data to other individuals or organisations, third parties such as doctors or agree to such disclosures. 

  • When you request us to process your personal data, such as ethnic, genetic, biometric, health, sex life or sexual orientation data.

  • You have the right to withdraw your consent at any time.  We will inform you if this affects our ability to provide healthcare/holistic services.

Sharing of your personal data:

  • Subject to data protection laws, we may share your personal data with consultants, doctors, or other healthcare professionals in an emergency or to protect your vital interests.

  • All information is kept safe and confidential.

  • Sharing of your personal data for marketing purposes:

  • We may send you special offers and promotions using your contact details with your consent.  We don’t sell your personal information to third parties.  You can opt-out anytime by contacting us at hello@to-nirvana.com.  This won’t affect the healthcare/holistic services we provide.

Retention Time

Your data is held for 7 years for legal and insurance purposes unless you are no longer a client of “to-Nirvana”.  You have the right to access your data, have data corrected if inaccurate, object to the processing of your personal data, receive your data and transfer it to another controller, or request its erasure.  However, if you request to have your data erased we will no longer be able to carry out any treatments for you. 

  • You have the right to file a complaint.
